*Surgical Hemorrhoid Treatments

Surgical procedures are usually reserved for treating painful clots, hemorrhoids that bleed constantly, acute, longstanding hemorrhoids that fail to respond to other treatments. Used on third and fourth degree hemorrhoids and patients who have serious problems with hemorrhoids skin tags.

Surgery provides better long term results but is more painful, expensive and recovery takes longer, surgical complications are also more common.

The most commonly used surgery types:

  • Lord dilatation - forceful dilation of the anal sphincter is used to stretch the anal canal, and weaken the anal sphincter. It is done because the increased sphincter pressure is the cause of hemorrhoids.
  • Dilatation is rarely used because it frequently causes damage to the sphincter itself, which results in many patients becoming incontinent and unable to control their bowel movements.
    Hemorrhoidectomy - with this treatment both the internal and external hemorrhoids are cut out, the wounds may be sutured (closed technique) or left open to heal (open technique).
  • In case of external hemorrhoids vein responsible is identified and excised together with overlying skin and underlying veins to prevent return in the same spot again.
    Sometimes proctoplasty is also necessary, it removes prolapsed anal lining higher in the anal canal.
    Pain is severe, and sometimes complications like urinating difficulty, delayed bleeding and infection can occur. Recovery is usually 2 -4 weeks.
  • Stapled hemorrhoidectomy - is one of the latest hemorrhoid treatments, it doesn't remove the hemorrhoids but it removes supporting tissue which has caused prolapse in the first place.
    It is much faster then traditional hemorrhoidectomy, causes less pain and patients can return to work earlier.
    Difficulty with this treatment is that it can cause infection, bleeding, anal fissures and narrowing of the anal canal.
